This section "How To" has been specially made for you dear adherent in order to help you and facilitate for you the procedures: - Keep your CML Hospitalization Card with you to benefit from our services.
- Follow the list of Hospitals, Labs and physicians enclosed in your contract
| In case of any state of emergency: | | ---------------------------------------------------------- | Call us on our Hotline 03/770533 that we have put at your service 24 hours per day before heading to the hospital. We will provide you with full medical information related to the emergency case and the procedures that you should follow. When you arrive to the hospital, send us the physician’s report by fax on 09/224010 | | In case of Non-Urgent operation | | ---------------------------------------------------------- | After you send us the physician’s report, your policy number, your date and place of operation and a copy of your hospi card , we shall study it according to your contract and according to the medical case. When the medical file is completed you shall receive our approval. If you can’t forward you physician’s report by fax, call us on 03/469146 or on 09/224006/7 listed on your hospitalization on card, so we can send to your place a CML delegate to avoid any services delay. | | In case of External physical examination (Out Patients Exam) | | ---------------------------------------------------------- | Keep in hand your detailed physician’s report and the list of tests you are intending to take. When you get to the laboratory, hand in your physician’s report, your hospitalization card and a copy of your ID in order to take all the tests required. To make things easier for you, CML conveys Laboratory Assistants to your place to make the tests, and delivers the results to your home the next day. |
